How to Check-in

INITIAL (FIRST) VISIT

Fill out and submit ‘Application Form for New Patient / Registered Information Change Notification and Consent Form’ with your health insurance card to INFORMATION(Sogo Annai 総合案内)for New Patient. This is to issue your hospital ID card and create medical record. If you arrive before 8:00 a.m., obtain numbered ticket at INFORMATION.

FORGOT TO BRING YOUR HOSPITAL ID CARD

Go to Window 1 – Return visit.(We wil check-in after confirming your name and date of birth)
*Night, Holidays, Saturday afternoon: Go to Window 3 – Accounting / After-Hours Reception

WITH YOUR HOSPITAL ID CARD
and have an appointment or visited our hospital within 3 months

Check-in by yourself with Reception Machines.

WITH YOUR HOSPITAL ID CARD
but did NOT visit our hospital …
a)more than 3 months to 1 year

Go to Window 1 – Return visit

b)more than 1 year

Fill out ‘Application Form for New Patient / Registered Information Change Notification and Consent Form’ then submit it to Window 1 – Return visit.
*Night, Holidays, Saturday afternoon: Go to Window 3 – Accounting / After-Hours Reception

Making an Appointment

 There are three ways to make an appointment.

  1. Talk to your doctor during consultation
  2. Go to Appointment Center(Yoyaku Center 予約センター*)
  3. Call Appointment Center(*)

* Making an appointment may not be available at some departments.

Numbered Ticket

Numbered ticket for the day is available at ticket machine from 6:00 a.m.
Reception Machines start operating from 8:00 a.m. and patients with numbered ticket check-in first.
Obtain numbered ticket. It is necessary for initial visit and/or with a referral letter, too.

The First Visit

Certificate Verification is Required

  1. To have definite identification assured by portrait.
  2. To protect patients from serious medical accidents caused by misidentification.(e.g. mismatching blood type; confused allergy information)
  3. To present precise statistics of nationality and language of foreign patients to Japanese government as the obligation of accredited medical institution accepting international patients.
  4. To prevent identity fraud by following the guidance given by Tokyo Regional Immigration Services Bureau and Chiba Prefectural Police regarding visa status confirmation.

Additional Fee for a Patient Without a Referral

Apart from the actual medical expenses, we will also charge the patient below an additional fee for a patient without a referral.
We appreciate your cooperation.

INITIAL VISIT

Medical department: 7,000 Yen(Tax included) Oral Surgery department: 5,000 Yen (Tax included)

Applies to

*Patients without a referral letter from other medical institutions at the initial visit
*Patients who visit another department without a referral leter during the visit on a regular basis(not applicable if you are referred from other medical department of our hospital)

RETURN VISIT

Medical department: 3,000 Yen(Tax included) Oral Surgery department: 1,900 Yen (Tax included)

Applies to

*Patients who wish to continue visitng our hospital instead of accepting our suggestion to visit other medical institutions with our referral
*Patients who visit another department without a referral leter during the visit on a regular basis(not applicable if you are referred from other medical department of our hospital)

We do NOT charge the fee(Additional Fee for a Patient Without a Referral)if the patient…

*had an emergency consultation by taken into our hospital by ambulance, and others
*admitted to our hospital immediately after outpatient visit
*referred from other medical department of our hospital
*is currently joining clinical trials, or had industrial accident, civil service accident, health checkup, thorough health checkup, and others
*entrolled in publicly-funded medical care programs for specific disability and/or disease

What is ‘Additional Fee for a Patient Without a Referral(Sentei Ryouyouhi 選定療養費)’?
A

This is a cost(fee)that the patient without a referral letter needs to pay, according to the system designated by Ministry of Health, Labour and Welfare of Japan. The purpose of this system is to assign and clarify the roles of medical institutions, i.e., primary care at a local hospital / clinic, advanced and specialty care at a larger hospital.

Our hospital(Regional Medical Care Support Hospital with more than 200 beds)is obliged to charge the fee.
